April 2010 April 26, 2010 - Knowledge Transfer & Exchange SessionIn partnership with the Canadian Cancer Society, the Toronto Central Palliative Care Network will host its first Knowledge Transfer & Exchange session. This session serves to give all those interested an understanding of the general concepts and principles of hospice palliaitive care, and will speak to how hospice palliative care is organized in the Toronto Central region. For more information, click here May 2010 May 2-8, 2010 - National Hospice Palliative Care WeekNational Hospice Palliative Care week is a one-week campaign that focuses on raising awarness about hospice palliative care. It is also a time to celebrate, recognize, and share the achievements of hospice palliative care throughout the nation. TCPCN members have generously offered to sponsor and host a series of educational and awareness sessions withint the Toronto Central LHIN related to this year's Hospice Palliative Care Week's theme: patient and caregiver experiences. This spring, Alliance Hosipce, Hospice Toronto, Perram House, and Philip Aziz Centre will join forces to host this year's 8th Annual Toronto Hike for Hospice Palliative Care to raise awareness and money in support of Toronto's community hospices. For more information, click here
Come join the Network as for its 2nd Annual State of the Union where an address will be given summarizing the success to-date and future directions of the TCPCN and hospice palliative care in the Toronto Central region. The main topic for this year's event is Integration. The event will feature a morning plenary session and an afternoon workshop where speakers and the audience will share their views and opinions on health services integration and hospice palliative care. For more information, click here June 2010 June 14, 2010 - Quality Education WorkshopThe purpose of the workshop, the first of a series of Lunch 'n Learns hosted by the Network this year, will be to raise our collective understanding of what is meant by quality and its role and application in hospice palliative care, and how we can develop leadership in this area. The workshop will be given by the Centre for Health Quality Improvement who have been assisting the province in much of the quality and integration work (and connected with the Change Foundation). The session will provide an overview of principles and concepts behind quality, the link that safety has to quality, identify some problem identifying and quality improvement tools, and discuss how to nurture leadership in quality among staff, management and board of directors. For more information, click here June 16, 2010 - Prognostication in Non-Cancer Populations The Toronto Central Palliative Care Network is pleased to present a session that examines prognostication in non-cancer populations. This will include a special focus on patients with congestive heart failure (CHF). Our session will examine the literature on the effectiveness of current prognistication practices for non-cancer patient populations. For more information, click here July 2010 July 23, 2010 - Life in the City: Trends in Diversity, Housing, Aging, & Living In partnership with the City of Toronto, the Toronto Central Palliative Care Network is pleased to present an intimate session where Mr. Mat Krepicz (Senior Analyst, Social Policy Analysis & Research, City of Toronto) will provide us with an introduction to the demographics of the City of Toronto with an emphasis on recent trends and possible future developments. For more information, click here August 2010 September 2010 September 23, 2010 - Homelessness & Palliative Care The Toronto Central Palliative Care Network is pleased to present a session that will focus on the issues and challenges of homelessness and hospice pallaitive care within the city of Toronto. The session will be kicked off with an update on World Hospice Palliative Care Day activities and a brief snapshot of the status of hospice palliative care worldwide. October 21, 2010 - International Series: "Hospice Palliative Care in Malawi" The Toronto Central Palliative Care Network is pleased to present a session where Dr. Sandy Buchman (Regional Primary Care Lead, Toronto Regional Cancer Program, Cancer Care Ontario) will speak to how hospice palliative care is delivered in Malawi, the successes and challenges in hospice palliative care, and thoughts on the necessary drivers to advancing such care in Malawi. For more information, click here December 2010 December 7, 2010 - Residential Hospices Update Forum The Toronto Central Palliative Care Network hosts a forum to provide members/partners and friends with an update on news for two new residential hospices in Toronto Central LHIN - The Kensington Hospice will be a 10 bed hospice, which will open in 2011 and The Children's Hospice. Executive Directors Bill O'Neill, Kensington Health Centre and Rauni Salminen, Philip Aziz Centre will provide an update on the progress and development of their respective residential hospices and be available to answer your questions including those around the clinical team, processes and procedures on access and other items of interest.
