"Innovations & Best Practices in Hospice Palliative Care"

Light breakfast & Lunch Provided

Participate in a free educatino day for hospice pallaitive care service providers (RNs, RPNs, PSWs) working in Toronto and the GTA. Interactive workshops will target hospice pallaitive care issues in Long-Term Care, Hospital, and Community settings.

Participants can:

  • Increase communication skills
  • Explore the beliefs and values of hospcie palliative care
  • Develop skills to address the psychosocial needs of patients and families
  • Receive an overview of regional hospice palliative care services for Central East, Toronto Central, Central, & Missisauga/Halton

This session is open to the entire network, their staff, board members, partners, and friends.We welcome and encourage all to come!

Speaker Biographies

Ms. Jocelyn Brown, RN, BA, BScN, MN

Palliative Care Clinical Nurse Specialist Princess Margaret Hospital, University Health Network

Jocelyn has been working at Princess Margaret Hospital as a Palliative Care Clinical Nurse Specialist since April 2009. This role includes providing clinical consultation to palliative care patients on acute care wards and education to staff.  She recently joined the de Souza Institute as an educator specializing in palliative care. Jocelyn also works in the Cancer Pain Clinic at PMH. Specific areas of interest include improving the palliative approach to care for the hematology oncology population. Prior to this she worked as an RN on both Malignant Hematology and Palliative Care units at PMH.  She graduated from University of Toronto with her Masters in Nursing in April 2009.

Dr. Mary Jane Esplen is a Clinician-Scientist and Professor, Department of Psychiatry, Faculty of Medicine, University of Toronto and Director of the de Souza Institute.
Dr. Esplen is a therapist and researcher working in psychosocial oncology and teaches at the University of Toronto. She has as strong interest in developing measurement tools and support groups or therapy interventions for cancer genetics populations and individuals with cancer. Dr. Esplen has an interest also in women’s health issues and works with individuals in the area of body image and sexual health.  Dr. Esplen is the recent past President of the Canadian Association of Psychosocial Oncology.

Ms. Tracey Human, RN
Consultant, Palliative Pain & Symptom Management Consultation Service for Toronto       Contact: thuman@dlhospice.org 

Tracey is a seasoned professional with 20 years of Palliative Care expertise.  Her 30 years of nursing encompassed high acuity units to community care, along with 13 years in the business sector delivering consultation on business development, programming, operational analysis, marketing and training.  Tracey is currently pursuing the CHPCN(C) designation and a Masters Certificate in Healthcare Management. 

Ms. Jennifer Land, RN, BScN, CHPCN (C)

Jennifer has worked as a Palliative Pain and Symptom Management Consultant since August 2009. Prior to assuming this position she was a Care Coordinator with the Dorothy Ley Hospice Community Program for 10 years. Jennifer is nationally certified in Hospice Palliative Care.
