Hospice Palliative Care

Knowledge Transfer & Exchange Session

Purpose:
This session will serve a number of purposes:

  • Give you an understanding of general concepts and principles of hospice palliative care, and will speak to some of the misconceptions (It is not only about end of life, or care for cancer patients).
  • Give you an understanding of how hospice palliative care is organized in the Toronto Central region
  • Provide you with information on who all the hospice palliative care providers are, their services, and how to access services for these different providers.

Audience:

  • For any worker and volunteer that is involved in providing care or making referrals to patients/clients who may benefit from hospice palliative care. 
  • This is open to any worker & volunteer in any healthcare & community support services organization including (all chronic disease management and care groups, community support services, services for homelessness, and others).

Speaker Biographies

Siu Mee Cheng, MHSc, BASc, CHE
Executive Director, Toronto Central Palliative Care Network

Siu Mee Cheng possesses over 13 years of experience in the public and non profit health sectors. She has engaged in many senior and leadership positions throughout her career including: Program Evaluation and Performance Measurement Lead for the Ministry of Health and Long Term Care, Senior Policy Advisor in Performance Measurement for the Government of Ontario, Director, Accountability and Risk Management for Cancer Care Ontario and lastly, Director for the Screening Action Group for the Canadian Partnership Against Cancer Corporation. Siu Mee also held an Instructor position for Ryerson University teaching fundamental research principles and theory and statistics. She sits on the governance council for the Red Cross, Toronto and is a Board Director for The Surrey Place Centre. She has also been appointed part-time vice-chair for the Workplace Safety Insurance Appeals Tribunal. Currently, Siu Mee occupies the position of Executive Director for the Toronto Central Palliative Care Network. She accepted the position in November 2008 and has provided significant leadership in moving the Network forward.

Karen De Prinse, RN, BScN, MN, CPMHN(c)
Chief Nursing Executive & Director of Clinical Programs, Casey House Hospice

Karen de Prinse has been practicing as a Registered Nurse for over 24 years and has been the Chief Nursing Executive and Director of Clinical Programs for Casey House for over two years. Her current role sees Karen focused on professional practice, program development and continuous quality improvement initiatives. Karen has extensive leadership and clinical experience in fields focused on serving marginalized and disenfranchised clients in both community and hospital environments. Coupled with this is Karen’s love for education, which has been a focus of her various roles for over 15 years in academic, clinical and community practice environments.

 

Baycrest - Enriching Care, Enhancing Knowledge, Enlightening Minds

Lisa Lun, RN, BScN, MA(Ed), CHPCN(c)
Clinical Manager of Palliative Care Unit, Baycrest Hospital

Following her baccalaureate nursing degree at the University of Toronto, Lisa joined the Medical Oncology team at Mount Sinai hospital where she was a staff nurse for 6 years. During that time she was also a part-time nursing instructor at Centennial College for the Oncology Nursing Program.

Lisa also had a keen interest in the community sector and took a position as a case manager at the “previously known” Home Care Program of Toronto in the North York area in 1996 where she helped to initiate the Palliative Care case management delivery model. In 2001, Lisa received her Masters in Education from Central Michigan University, and joined the Cancer Care and Palliative Care program at Humber River Regional Hospital as a Nurse Clinician where she continued to provide case management support to patients and families. Since then, she has participated in many regional and LHIN committees, including Chair of the North Regional Palliative Care Committee for 5 years and an active participant at the Toronto Palliative Care Network and the Central Palliative Care Network.

Lisa has also been an active member of the Toronto In-Patient Palliative Care Unit committee for about 9 years. For the past 3 years, she has taken the lead for the Palliative Care Common Referral Form which was originally developed for the city of Toronto in 2004.

She achieved her Canadian Nurses Association certification in Hospice Palliative Care in 2005 and is currently a Clinical Manager for the Palliative Care unit at Baycrest Hospital.

 

Belinda Marchese
Director of Clinical Services, Hospice Toronto

Belinda Marchese holds over 22 years of experience in the social service and health care sectors.  Belinda has been actively working in the Hospice Palliative Care sector since 2005 and joined Hospice Toronto in September 2007.  She currently occupies the position of Director of Clinical Services at Hospice Toronto. Prior to this, Belinda had extensive clinical experience in Social Work, combined with providing progressive management to diverse programs and services that focused on children, families and adults across the Toronto area.  

 

Jeff Myers, MD, CCFP, MSEd
Head of Palliative Care Consult Team, Sunnybrook Health Sciences Centre
Co-Program Head – Patient & Family Support Program, Odette Cancer Centre

Dr. Jeff Myers is Head of Sunnybrook’s Palliative Care Consult Team, Program Head for the Odette Cancer Centre Patient and Family Support Program and the Toronto-Central LHIN’s Palliative Care Lead for Cancer Care Ontario.  He is an Assistant Professor and is currently Associate Head of University of Toronto’s Academic Division of Palliative Care.

 

Kim Pearson
Client Services Manager of Palliative Program, Toronto Central Community Care Access Centre

Kim Pearson is the Client Services Manager of the Palliative Program and Princess Margaret Hospital.  She has also held other portfolios in the CCAC including Child and Family and Quality and Contracts.

 

Sheila Berry
Case Manager, Alliance Hospice

Sheila Berry currently occupies the position of Case Manager at Alliance Hospice.

 

Ron Lirette
Executive Director, The Dorothy Ley Hospice & Perram House

Ron currently occupies the position of Executive Director for both The Dorothy Ley Hospice and Perram House.
