State of The Union 2010 *The Network would like to thank all those who attend our 2010 State of the Union. Presentations have been uploaded!! Please scroll to bottom!* Date: Friday May 28, 2010 Time: 8:30 - 3:30 PM Registration: 8:00 - 8:30 AM Location: FitzGerald building - Room 103 150 College Street, Toronto, ON Breakfast & Lunch Provided The Toronto Central Palliative Care Network is the forum where by regional stakeholders with an interest and focus in hospice palliative care come together to ensure integrated quality hospice palliative care within the Toronto Central LHIN. 2009-10 proved to be an exceptional year for the network and the region. A number of achievements were realized and significant momentum around coordination and collaboration was seen. Come join the network on May 28, 2010 from 8:30-3:30 for the second annual State of the Union. A State of the Union Address will be given summarizing the successes to-date and future directions. The event will also feature a plenary session where speakers, including the TC LHIN, will share their views and opinions on health services and hospice palliative care integration, including ideas around Aging At Home, wait times, Single Points of Access, Continuity of Care, Navigation, and Care Closer to Home. The region’s Hospice Palliative Care Systems Integration Blueprint & Plan will also be unveiled. A workshop will be planned for the afternoon, where the audience will be engaged to help identify actions that will help realize the vision of an integrated hospice palliative care system for the region. Staff and volunteers from Network member organizations and regional partners and friends are encouraged to attend. This will be a great opportunity to network and engage in dialogue with your colleagues and community and healthcare partners, and to hear about the progress that has been made to-date and future directions in hospice palliative care in the region KEY PRESENTATIONS

S. Lawrence Librach, MD, CCFP, FCFP Dr. Librach’s involvement in Palliative Care began in 1978. Dr Librach is now the W. Gifford-Jones Professor in Pain Control and Palliative Care at the University of Toronto, where he also holds the position of full professor in the Department of Family and Community Medicine. Dr. Librach is also an associate in the Joint Centre for Bioethics and the Centre for Studies in Pain at the University of Toronto and the Buehler Centre for Aging at Northwestern University in Chicago. He was the physician leader of the national Educating Future Physicians for Palliative and End of Life Care Project (EFPPEC) and education consultant to the Education in Palliative & End of Life Care (EPEC) project in the USA. He is a member of the Board and Past President of the Canadian Hospice Palliative Care Association.
Siu Mee Cheng, MHSc, BASc, CHE Siu Mee Cheng possesses over 13 years of experience in the public and non profit health sectors. She has engaged in many senior and leadership positions throughout her career including: Program Evaluation and Performance Measurement Lead for the Ministry of Health and Long Term Care, Senior Policy Advisor in Performance Measurement for the Government of Ontario, Director, Accountability and Risk Management for Cancer Care Ontario and lastly, Director for the Screening Action Group for the Canadian Partnership Against Cancer Corporation. Siu Mee also held an Instructor position for Ryerson University teaching fundamental research principles and theory and statistics. She sits on the governance council for the Red Cross, Toronto and is a Board Director for The Surrey Place Centre. She has also been appointed part-time vice-chair for the Workplace Safety Insurance Appeals Tribunal. Currently, Siu Mee occupies the position of Executive Director for the Toronto Central Palliative Care Network. She accepted the position in November 2008 and has provided significant leadership in moving the Network forward.
Wendy Sutton, BA, LL.B, LL.M Wendy is a Toronto lawyer (Osgoode Hall Law School: LL.B.1997; LL.M, 2001 (health law)) and was called to the Bar in 2000. In May 2009, Wendy was appointed Vice Chair of the Ontario Workplace Safety and Insurance Appeals Tribunal. Since 2007, she has been an Independent Security Advisor to the Minister of Transport on the Passenger Protect Program and the Marine Transportation Security Clearance Program. Wendy served as legal Chair of the Canada Pension Plan/Old Age Security Review Tribunal (2004 – 2007) and as a public member of the Interim Regulatory Council on Midwifery (1989 – 1992). She was Deputy Registrar (Acting) and Director, Professional Practices for the College of Dental Technologists of Ontario (2006 – 2009). Wendy is an Affiliate Member, York University Centre for Health Studies and Associate Director of Osgoode Hall’s part-time LL.M. in Health Law (2004 -). She is co-author of “Preventing Sexual Abuse of Patients” - a legal guide for health care professionals (with Marilou McPhedran, 2004). Wendy has served on numerous Boards, including the Michener Institute of Applied Health Sciences (Chair); Toronto Birth Centre, Inc. (President), Women’s College Hospital, Sunnybrook Hospital and 416 Drop-In Centre, Wendy received the City of Toronto Volunteer of the Year Award (2004).
A. Paul Williams, PhD Paul has taught and published nationally and internationally on health systems and health policy, with emphasis on the role of community-based services in maintaining the health and well-being of seniors. He was Co-Director of M-THAC and is the Co-Chair of the Canadian Research Network for Care in the Community (CRNCC). He is also Co-director of the CIHR Team in Home and Community Care and Health Human Resources, where he leads research examining the "balance" of institutional and community-based care for seniors, children and ethno-racial communities.
Janine Hopkins, MA Janine Hopkins has 17 years of experience in public affairs and health policy in the public, not-for-profit and private sectors. Janine leads the Toronto Central LHIN's community engagement and communications strategy and initiatives and provides executive support to the LHIN's Board of Directors.
Raymond Berry, MD Dr. Berry is the currently the attending physician on the palliative care unit at Toronto Rehabilitation Institute where he is the Medical Director of Long Term Care and also serves as Chair of the Medical Advisory Committee. He is an Assistant Professor in the Department of Family and Community Medicine, University of Toronto, and obtained certification from the American Board of Hospice and Palliative Medicine in 2007. He is a member of the Executive Committee of the Toronto Central Palliative Care Network and chairs the network’s Integration and Planning Subcommittee.
