This year's Toronto Central Palliative Care Network's annual State of the Union event promises to be the most exciting and stimulating event to-date. The first of its kind in the province, the Toronto Central Palliative Care Network in partnership with the Alzheimer Society of Toronto, Bridgepoint Health, and the Ontario Lung Association cordially invite you to attend this year's themed event focused on discussing the issue of hospice palliative care (HPC) for non-cancer chronic diseases (heart disease, stroke, COPD, and Alzheimer's disease).
The current regional and provincial HPC systems are not oriented to optimally respond to the needs of non-cancer and chronic disease patients. Access and quailty of care issues are some of the pervasive challenges. The TCPCN and its regional and provincial partners recognize this as a key and current theme.
Join us on June 3, 2011 in Toronto and hear from some of the province's and country's thought leaders. You will gain new insights and challenge existing ideas on HPC and chronic diseases. Furthermore, the regional and provincial chronic disease-focused partners and friends will discuss what they are doing in HPC and where improvement prospects may lie. There will be opportunities for you to dialogue with the expert speakers and your colleagues in a Q&A panel and break-out sessions.
This is a not-to-be missed event for any health professional and volunteer who has an interest in chronic diseases, hospice palliative care, or health care in general. We welcome and encourage all to attend!!

Speaker Biographies
S. Lawrence Librach, MD, CCFP, FCFP
Chair, Executive Committee & Quality of Care Committee, Toronto Central Palliative Care Network
Director, The Temmy Latner Centre For Palliative Care, Mount Sinai Hospital
Dr. Librach’s involvement in Palliative Care began in 1978. Dr Librach is now the W. Gifford-Jones Professor in Pain Control and Palliative Care at the University of Toronto, where he also holds the position of full professor in the Department of Family and Community Medicine. Dr. Librach is also an associate in the Joint Centre for Bioethics and the Centre for Studies in Pain at the University of Toronto and the Buehler Centre for Aging at Northwestern University in Chicago. He was the physician leader of the national Educating Future Physicians for Palliative and End of Life Care Project (EFPPEC) and education consultant to the Education in Palliative & End of Life Care (EPEC) project in the USA. He is a member of the Board and Past President of the Canadian Hospice Palliative Care Association.
Dr. Librach now serves as the Director of The Temmy Latner Centre for Palliative Care at Mount Sinai Hospital. This Centre includes a Home Palliative Care Program, an Inpatient Palliative Care program, a Psychosocial-Spiritual Program, a Children’s Program, and academic programs in education and research.
Marian Walsh, BSW, MBA
President & Chief Executive Officer, Bridgepoint Health
Marian Walsh is President and CEO of Bridgepoint Health. Identified as “a futurist” by Healthcare Manager Magazine., she is a visionary and has been an effective advocate for transforming the health care system to meet the evolving needs of patients for over 30 years. At Bridgepoint, Marian has built Canada’s leading healthcare organization for research, education and care focused on complex chronic disease and disability. Currently, she is overseeing redevelopment of the new Bridgepoint Hospital, a state of the art, purpose built, 680,000 sq. ft. facility scheduled to open in 2013. Prior to Bridgepoint, Marian held leadership and advisory roles that have greatly influenced understanding and the direction of complex chronic care in the Province and across Canada. In addition to her responsibilities at Bridgepoint, she was interim President and CEO at Women’s College Hospital. Previously she was President and CEO of Toronto’s Home Care Program, a Special Advisor to Newfoundland’s Ministers of Health and Social Services, and CEO of St. John’s Regional Home Care Program (NFLD.). Her national leadership roles include Founding President, Canadian Home Care Association, President, Canadian Association of Social Workers, and Canadian Representative, International Federation of Social Welfare. She plays an active role in the Ontario Hospital Association, the Rehab Network of Toronto, and the Complex Care and Rehabilitation Leadership Council of Ontario. She is also a member of the Advisory Committee, Department of Health Policy, Management and Evaluation, University of Toronto, a member of the Board of Medavie Blue Cross, and is a Certified Health Executive.
Wendy Sutton, BA, LL.B, LL.M
Vice Chair, Workplace Safety & Insurance Appeals Tribunal
Wendy is a Toronto lawyer (Osgoode Hall Law School: LL.B.1997; LL.M, 2001 (health law)) and was called to the Bar in 2000. In May 2009, Wendy was appointed Vice Chair of the Ontario Workplace Safety and Insurance Appeals Tribunal. Since 2007, she has been an Independent Security Advisor to the Minister of Transport on the Passenger Protect Program and the Marine Transportation Security Clearance Program. Wendy served as legal Chair of the Canada Pension Plan/Old Age Security Review Tribunal (2004 – 2007) and as a public member of the Interim Regulatory Council on Midwifery (1989 – 1992). She was Deputy Registrar (Acting) and Director, Professional Practices for the College of Dental Technologists of Ontario (2006 – 2009). Wendy is an Affiliate Member, York University Centre for Health Studies and Associate Director of Osgoode Hall’s part-time LL.M. in Health Law (2004-). She is co-author of “Preventing Sexual Abuse of Patients” - a legal guide for health care professionals (with Marilou McPhedran, 2004). Wendy has served on numerous Boards, including the Michener Institute of Applied Health Sciences (Chair); Toronto Birth Centre, Inc. (President), Women’s College Hospital, Sunnybrook Hospital and 416 Drop-In Centre. Wendy received the City of Toronto Volunteer of the Year Award (2004).
Jocelyn Charles, MD, CCFP
Medical Director, Veteren’s Centre, Sunnybrook Health Sciences Centre
Dr. Jocelyn Charles is Medical Director of the Veterans Centre at Sunnybrook. She is also Chief of Family & Community Medicine at Sunnybrook and Associate Professor in the Department of Family and Community Medicine at the University of Toronto. Dr. Charles has worked in the Cognitive Support Unit in the Veterans Centre for the past 21 years and she is currently the attending physician in the Dorothy Macham Home, a 10-bed home at Sunnybrook for Veterans with challenging behaviours due to dementia.
Marija Padjen,
Chief Program Officer, The Alzheimer Society of Toronto
Marija Padjen has devoted most of her career to ensuring individuals and families touched by Alzheimer's disease and related dementia maintain quality of life for as long as possible, as well as raising awareness about the disease. She started with the Alzheimer Society of Toronto in 1998 and is currently the Chief Program Officer. In her role, she is responsible for developing and facilitating a broad range of educational and support programs to guide people through the different stages of the disease, including complex life-altering decisions. After graduating from McGill University in Montreal with a Bachelor of Arts, Marija moved on to York University in Toronto to earn both Bachelor and Masters degrees in Social Work.
Patricia Strachan, B.Sc.N., M.Sc.N., Ph.D.
Assistant Professor, School of Nursing, McMaster University
Patricia Strachan first joined McMaster University in 1986 as a Staff Nurse in the Intensive Care Unit. She is currently an Assistant Professor in the School of Nursing. She has completed a Post-Doctoral Research Fellowship with the Heart and Stroke Foundation of Ontario and a Strategic Training Fellowship in the FUTURE Program for Cardiovascular Nurse Scientists.
Patricia has a Bachelor of Science in Nursing from McMaster University, a Master of Science from the University of Guelph, and a Doctor of Philosophy from McMaster University. She has been a Principle Investigator on CIHR funded projects and a co-investigator on CIHR and HSFO research projects. Her primary research interests include heart failure, end of life care, communication and rural health.
Patricia is a member of the Advisory Board for the Cardiac and Vascular Nursing Science Unit at Hamilton Health Sciences.
Dylan Blacquiere, MD
Chief Resident, Adult Neurology, University of Ottawa
Dylan Blacquiere is the chief resident of the Neurology program at the University of Ottawa, where he has been training since completing his medical degree at Dalhousie Medical School in 2007. He has been interested in palliative care and stroke since his undergraduate studies, where he examined the provision of an organized approach to palliative care on an acute stroke unit, and how conflict occurred between families of patients and health care providers. He is currently working on a prospective study on how end-of-life care is given to patients with severe stroke, and with his colleagues, is completing a systematic review on palliative care and cerebrovascular disease. His work in this field has recently been presented at the 2011 International Stroke Conference in Los Angeles, California.
Krystyna Skrabka, RN
Regional Stroke Education Coordinator, South East Toronto Stroke Network
Krystyna Skrabka is a Registered Nurse and holds a Bachelor’s degree and Master’s degree in psychology. Krystyna has held both clinical and management positions in Long-Term Care, Community Care, Rehabilitation and Acute Care. She joined the South East Toronto Regional Stroke Network based at St. Michael’s Hospital in 2002 and currently holds the position of Regional Stroke Education Coordinator.
Jean Bourbeau, MD, MSc, FRCPC
Director, Respiratory Epidemiology and Clinical Research Unit, Montreal Chest Institute, McGill University
Jean Bourbeau is full time researcher in the Department of Medicine. His areas of research are in COPD (epidemiology and clinical/evaluative research); and in disease management, self-management, pulmonary rehabilitation and cardiorespiratory exercise physiology.
One of his major contributions has been the development of the self-management program “Living well with COPD” (website: www.livingwellwithcopd.comm; password: copd) and completing of landmark trials (RCT) in self-management and pulmonary rehabilitation. Many major clinical trials have been adapting and implementing the self-management program “Living well with COPD” in Canada and worldwide. His work, particularly in COPD, Pulmonary Rehabilitation and Self-Management in COPD has not had impact only in research but also in the clinical practice and the public domain.
Carole Madeley, RRT, CRE, MASc
Director, Respiratory Health Programs, Ontario Lung Association
Carole is the Director of Respiratory Health Programs, for the Ontario Lung Association. She is responsible for the delivery of five asthma programs funded by the MOHLTC as part of the Ontario Asthma Plan of Action. Her responsibilities in this position include marketing, media initiatives, The Lung Association's BreathWorks COPD programs and participating on several respiratory advisory committees. Carole graduated from The Michener Institute in Toronto as a Respiratory Therapist in 1981 and she holds a Master of Applied Science degree. Prior to working with the Ontario Lung Association, Carole worked for 9 years as the Team Leader in a respiratory rehabilitation program. She also worked for 26 years as an acute care RRT. Carole has presented respiratory related topics at numerous educational events and seminars. Carole has published articles in respiratory journals, in newspapers and she has been interviewed on radio and television.

Kim Pearson, Client Services Manager, Palliative Program, Toronto Central CCAC
Kim Pearson is the Client Services Manager of the Palliative Program and Princess Margaret Hospital. She has also held other portfolios in the CCAC including Child and Family and Quality and Contracts.
Jennifer Arvanitis, Physician, Temmy Latner Centre for Palliative Care, Mount Sinai Hospital
Jennifer Arvanitis graduated from the University of Western Ontario in 1993 and completed her residency there in family practice. Following 7 years of general family practice including emergency medicine, she decided to pursue extra training in palliative medicine. She has been a full time staff physician at the Temmy Latner Centre for Palliative Care at Mount Sinai Hospital since 2002, providing home-based end-of-life care to approximately 350 patients per year in the community. Dr. Arvanitis is a Lecturer at the University of Toronto and is actively involved in teaching medical students, residents, fellows and community physicians in the field of palliative medicine.
James Downar, Palliative Care & Intensive Care Medicine, University Health Network & Toronto East General Hospital
James Downar is a Critical Care and Palliative Care physician at the University Health Network in Toronto. He graduated from McGill University Medical School in 2002, and completed residencies in Internal medicine, Palliative Care Medicine, and Critical Care Medicine at the University of Toronto. He has also completed a Master’s degree in Bioethics at the Joint Centre for Bioethics at the University of Toronto. His academic interests include educating medical trainees about communication and decision-making with seriously ill patients and their surrogates.
